Point to the exact item

Include the date, visit or document, and the wording that seems incorrect. Explain what you believe is accurate and any supporting information. Avoid assuming that editing a personal profile field will amend a clinician's note or another linked system.

A practical example

The visit summary dated October 10 lists a procedure on the left side. The procedure was on the right. What is the process for reviewing and correcting that entry?

This hypothetical request gives the records team something specific to investigate.

Urgent care questions need a separate route

If an error could affect an imminent treatment decision, contact the treating team promptly as well as using the records process. An administrative amendment request may not be reviewed on the same timeline as a clinical concern.

Keep the response

In the United States, HHS explains that people can request amendments to records held by covered entities and may submit a statement of disagreement when a request is denied. The organization's process and the type of record matter.

After a correction, ask whether related displays or shared records will update. A corrected source note and an old downloaded copy can coexist, so the date and version remain important.
