Bring the uncertainty too
If you are unsure about a name or dose, the packaging or a pharmacy list may help. Avoid filling a gap with a guess. A clear statement that an entry needs checking is safer and more informative than an apparently complete but inaccurate list.
Include relevant nonprescription medicines and supplements when the care team requests a full review.
A compact preparation note
| Item | Useful detail |
|---|---|
| Medicine | Name as shown on the label |
| Current use | Dose, timing, and route if relevant |
| Recent change | What changed and who advised it |
| Question | The specific point needing clarification |
This is preparation for a conversation, not a recommendation to change treatment.
Make disagreements visible
If the portal lists something you no longer take, mention it rather than assuming the clinician knows. If two instructions conflict, ask which applies. A remote appointment can review discrepancies, but the display may not update immediately.
Before finishing, clarify any new instructions in your own words and ask where the updated plan will be documented. That gives both sides a chance to catch a misunderstanding while the conversation is still open.
